Community Integration Services (CIS)

Compassionate Case Management to Support Your Housing Journey

At Rainbow Health, we’re committed to helping you thrive—starting with safe, stable housing. That’s why we offer Community Integration Services (CIS), a Medicaid-reimbursable case management program for individuals experiencing homelessness or at risk of losing their housing.

Our CIS case managers are here to walk alongside you, offering:

• Personalized support to connect you with housing and health resources

• Guidance on navigating healthcare, housing, and social services

• Advocacy and follow-up to ensure your needs are met

• Coordinated care that centers your well-being and long-term stability

While CIS does not cover rent or utilities, we provide the pre- and post-tenancy support you need to find and maintain a home—including assistance with housing applications, landlord communication, and daily living skills.

Eligibility: CIS is available to eligible QUEST Integration members.
Currently accepted plans: ʻOhana Health Plan and AlohaCare.
